This presentation will explore opportunities to support high-quality, person-centred care in LTC while managing acute and complex clinical situations. We will review ED diversion and hospital admission avoidance, consider the benefits and risks of hospital transfer, and discuss practical approaches to safely assess and manage residents in LTC.

The goal is not to avoid hospital when it is needed, but to strengthen clinical decision-making and support care that aligns with resident safety, comfort, continuity, and individual goals, values and preferences.
