May 22nd Webinar:
Medication Incidents in Long Term Care
Guest Speakers: Christina Jeong and Dr. Michael Hamilton
Data from 2021 suggests that over 50% of long-term care residents in Canada are prescribed 10 or more medications, making drug prescribing, dispensing, and administration one of the most common tasks performed in long-term care. With this volume of activity, even small error rates can have large effects on resident care and quality of life and can impact already burdened staff. We'll explore what we know about medication errors in long-term care, look at some of the underlying science behind errors and error prevention, and introduce practical approaches and tools for improving safety and quality.
Learning Objectives:
-
Recognize the context and prevalence of medication incidents in LTC
-
Explore system and behavioral factors that contribute to incidents, and principles behind the mitigation of errors
-
Identify a selection of educational workshops, tools, and publications that can inform improvements in quality and safety at your home
